General and Digestive Surgery

On first consideration of the definition of General and Gastrointestinal Surgery (GGIS), it may be said that surgical problems whose clinical and technical analysis and solution correspond primarily to their field (as set out in the 1986 program and in others recently revised by the American Board of Surgery and the Association of Surgeons of Great Britain and Ireland, both in 1989) are those corresponding to the digestive system, the endocrine system, the mammary glands, the abdomen and its contents, the skin and soft tissues, and the head and neck, as sorted by organ system or anatomical area.

On second consideration of the definition of General and Gastrointestinal Surgery, it should be noted that the specialty subsidiarily encompasses the initial approach to and resolution of (up to a certain degree of complexity) urgent surgical problems that in principle fall within other surgical specialties (Vascular Surgery, Thoracic Surgery, Urology, Neurosurgery), when a general surgeon assumes responsibility for this care at an intermediate-level hospital, like the regional hospital in our healthcare organization.

In these circumstances, the general surgeon must possess the necessary expertise and take responsibility for this care, before transferring it, if necessary, and in the appropriate conditions, to a higher-level hospital at which the abovementioned surgical specialties are offered.

On third consideration of the definition of GGIS, this surgical specialty is characterized, and must continue to be characterized, by its special focus on the biological and technical foundations of surgery, a focus that gives it a broad basis for training in the principles of surgery, thus enhancing its potential versatility both for healthcare and for research and education.

These biological and technical foundations of surgery include: The general pathology of injuries; the biological process of healing traumatic injuries; the biological response to accidental and operative trauma; deviations in the biological response to trauma and single and multiple organ failure, associated with post-operative complications intensive care in surgery patients; surgical infections and surgery for infections; immunology and surgery; nutrition and surgery; the biological and technical bases of tissue and organ transplants; and the principles of Surgical Oncology.
